Case Manager

Reports To: Director of Case Management and/or Case Management Supervisor

Position Summary: The Case Manager provides trauma informed and housing focused case management services to participants in various emergency housing, safe haven, and supportive housing programs. The Case Manager provides direct service links to external resources necessary to establish safe and affordable permanent housing with a goal to end participant’s homelessness and prevent a return to homelessness. All responsibilities are implemented according to our organizations mission and standards along with guidance from our funders.

Key Duties and Responsibilities

  • Provide case management services to a caseload typically ranging from 25-35 participants depending on assigned program location.
  • Meet and perform initial assessment of each participant assigned to your caseload with 72 business hours of their enrollment into the program.
  • Identify, screen, arrange, schedule and develop plans to connect participants to needed alcohol, drug, behavioral health, mental health, legal and/or medical services.
  • Research and maintain, current information on resources, benefits and services for which program participants may be eligible.
  • Provide information and professional level case management services to participants on a consistent and continual basis.
  • Maintain awareness of the health, welfare and treatment status of each participant on the assigned caseload.
  • Assist participants with obtaining identification (i.e. birth certificates, state identification, and social security cards).
  • Develop individualized service plan with agreed upon time lines for completion of approved goals.
  • Provide counseling and/or training on life skills to help ensure that participants are prepared for independent living.
  • Enter participant related data into Office of Homeless Services Homeless Management Information System (HMIS) and other required data management systems without mistakes and in a timely manner.
  • Complete and submit housing assessments and applications for participants as required by funder guidance.

  • Develop and maintain participant case records, compile homeless history, maintain accurate and complete documentation of case management services, interaction and participant progress.
  • Participate in Exchange Meetings, Participant Progress Reviews, and collaborative meetings with external agencies working with program participants, providing information.
  • Participate in all required trainings, team meetings and/or information meetings.
  • Meet all requirements for submitting required reports to in a timely manner (i.e. weekly work schedule, monthly services report, housing interventions).
  • Coordinate efforts closely with community and supportive service agencies working with program participants, and documenting all communication.
  • Serve as advocate for participants. Respond with flexibility in cooperation with requests to perform tasks directly related to providing services to program participants which may entail changes in work schedule or assignments to ensure that participants receive services.
  • Oversee the day to day program operations in the absence of the Program Manager or Residential Aides to ensure that shelter services are delivered as contracted. This may include: administering medications to participants, inspecting participant rooms to ensure they are in compliance with rules and regulations regarding items permitted on site, cleanliness, and respecting the privacy of others, and ensuring that participants’ meals are provided in a timely and safe manner.
