Case Manager

Are you looking for a meaningful career where every day you have the opportunity to make a lasting impact on vulnerable members of the community? Senior Friendship Centers is seeking a compassionate, organized, and self-motivated Case Manager to join our team.

In this role, you'll work directly with older adults throughout the community, assessing their needs, developing individualized care plans, and connecting them with essential services that help them remain safe, healthy, and independent in their own homes. This position meets with seniors in their homes to complete assessments in person. Other related work can be completed remotely from home. Candidates must be comfortable driving to multiple locations using their personal vehicle. This is a remote work opportunity for applicants who live within commuting distance of Lee County. To better serve our community, we are searching for someone bilingual in Spanish/English.

The ideal candidate is someone who genuinely enjoys helping others while living out our core values of compassion, integrity, respect, and collaboration. The Case Manager is a full-time Monday-Friday position. We offer a robust benefits package including health, dental, and vision coverage; generous paid time off; paid holidays; a 403(b) retirement plan with employer match; and—most importantly—the chance to do meaningful work with an organization that truly cares. If you’re ready to make an impact, we’d love to hear from you—apply today!
