  2. Coordinates with: Residential Services Coordinator, Professional Staff, Direct Support Professionals, Consultants, Guardians

V.SPECTRUM OF SOLUTIONS OBJECTIVES

To provide resources and efficient programs to address the needs & empower the lives of individuals with IDD, to assess and identify the continued and developing needs of the IDD community, to advocate on local, state, and federal levels the rights of individuals with IDD, to promote education through community learning sessions and events, to effectively inspire, recruit, maintain and appreciate volunteers, to secure long-term financial support to fund established priorities.

VI.QUALIFICATION REQUIREMENTS

  1. High school diploma/equivalent required
  2. 1-2 years of experience in an equivalent role preferred
  3. Must be able to work in an office (not a remote role) and travel for company business as needed
  4. Valid Texas driver’s license and related insurance
  5. Knowledge, skills, and abilities (KSA):
    1. Knowledge of inventory and supply ordering, distribution, and control processes
and practices
  1. Proficient computer skills and ability to manage inventory, supply ordering, and accounting/cost software
  2. Proficient ability to maintain strict confidentiality
  3. Strong communication, interpersonal, organizational, and time management skills
  4. Strong attention to detail, ability to maintain precise standards, and follow established processes
  5. Strong critical thinking and analytical skills
  6. Ability to work independently or as part of a team

VII.ESSENTIAL JOB FUNCTIONS

The Case Manager is responsible for coordinating the development and implementation of each
person’s programs, various support services, and training for people with intellectual disabilities.

Essential job functions include, but are not limited to the following key areas:

  1. Coordinate program plans by leading meetings and documenting goals and ensure annual review of plans.
  2. Deliver program plans by placing training documents in service locations and training staff on implementation and data collection.
  3. Monitor goals by overseeing staff interactions and data collection.
  4. Integrate services by finding community services that accept Medicaid.
  5. Record progress by directing staff in data collection and reviewing, and adjusting goals based on progress.
  6. Communicate with families and agencies.
  7. Develop pre-discharge and pre-admission plans.
  8. Ensure compliance with guidelines.
  9. Submit paperwork to the billing department.
  10. Maintain client eligibility.
  11. Maintain therapeutic relationships by encouraging family involvement, monitoring living and work settings, and ensuring community integration.
  12. Provide direct care as needed.
  13. Maintain good relationships with staff.
  14. Ensure programming meets standards.
  15. Cover for other Case Managers.
  16. Other duties assigned

VIII.WORKING CONDITIONS

The working conditions for this position include working in a climate-controlled office and residential environment with minimal safety hazards and travel between company locations, including residences. This position requires the ability to care for individuals with intellectual disabilities, work in a fast-paced environment with strict deadlines, follow policies/procedures, multitask, and perform complex/varied/simple/repetitive tasks with precise attention to detail. Must be able to control the work pace, follow instructions, influence people, handle confrontational situations, and work alone or in a group. The position involves working with
confidential client information and may require irregular hours and travel to other locations for company needs. Tobacco-free environment.

IX.PHYSICAL REQUIREMENTS

The physical requirements for this position are considered to be low and require frequent walking, standing, sitting, bending forward, reaching and lifting above shoulder level, below chest level, and/or chest-to-shoulder level, elbow extension, wrist extension and rotation, twisting and tilting of the neck, and also occasional squatting and kneeling. The individual must have fine motor skills to operate office equipment such as computers, phones, and printers. Sensory demands include hearing, speech, color vision, near vision, spatial perception, tactile, and smell.
