Position Summary
GateHouse Treatment is seeking a Case Manager to join our multidisciplinary treatment team. Reporting to the Clinical Supervisor, the Case Manager works directly with clients throughout their treatment experience, helping them identify needs, access resources, build practical life skills, and prepare for continued recovery after discharge.
This is a hands-on, client-focused role for someone who is organized, compassionate, and comfortable working as part of a collaborative behavioral health team.
What You’ll Do
- Provide case management services based on each client’s individual treatment plan and needs.
- Connect clients with community resources, medical providers, housing assistance, employment services, recovery supports, and other needed services.
- Work closely with clinical, medical, housing, and operations staff to coordinate care.
- Help clients develop realistic aftercare and discharge plans beginning early in treatment.
- Assist clients with identifying recovery goals, addressing barriers, and building skills that support long-term stability.
- Maintain accurate and timely documentation in the electronic health record (EHR).
- Assist with client intake, orientation, and coordination of initial service needs.
- Engage family members or other supports in the treatment and discharge-planning process when appropriate.
- Facilitate assigned psychoeducational, life-skills, or recovery-focused groups.
- Participate in treatment team meetings and provide updates on client needs, progress, and discharge planning.
- Recognize and communicate client concerns or changes that may require additional clinical support.
- Maintain appropriate professional boundaries and protect client confidentiality.
Requirements
Required:
- High school diploma or equivalent.
- Strong communication and interpersonal skills.
- Ability to work professionally and compassionately with individuals receiving substance use treatment.
- Basic computer skills and ability to complete accurate electronic documentation.
- Ability to collaborate effectively with a multidisciplinary team.
Preferred:
- Bachelor’s degree in human services, behavioral health, social services, or a related field.
- Certified Recovery Support Worker (CRSW) credential.
- Previous experience in substance use treatment, behavioral health, case management, recovery services, or a related setting.
- Familiarity with substance use disorders, recovery models, and community-based resources.
