Case Manager/Swing Bed

The Case Manager and Swing Bed Coordinator leads discharge planning, utilization review, care coordination, and the Swing Bed program. The position works with patients, families, providers, nursing, therapy, community partners, and payers to promote appropriate admissions, safe transitions, efficient throughput, and continuity of care.
