Case Manager - Transitional Housing & Rapid Rehousing
NewBe an early applicantUnder general supervision of Emergency Housing Services department, and working collaboratively with other Youth Homelessness Demonstration Program agencies, provides case management services to homeless and formerly homeless young adults (18-24) utilizing the Transition in Place housing model to facilitate rapid exit from shelter, using evidenced based best practices such as Trauma Informed Care and Motivational Interviewing. Develops and implements the goals and objectives to maintain housing and meet the individual’s identified goals, including but not limited to decrease in distress caused by mental health symptoms and substance use issues, improved interpersonal relationships, increased self-sufficiency, and increased income. Recertifies program eligibility, makes home-visits, and follows-up with participants to assist in maintaining their housing and independence
This is a non-exempt, union position with a pay rate of $19.00 per hour. The hours of work are Monday through Friday, 8:30am – 5:00pm.
