POSITION SUMMARY

The CDCA Case Manager provides patients of Basecamp Recovery Center with comprehensive, recovery-oriented case management services to support sustained recovery and overall wellbeing. This role is responsible for developing and implementing individualized case management plans, connecting patients to community resources, and serving as an advocate to ensure each patient has access to the services and supports they need to thrive.

The CDCA Case Manager works as part of BRC's clinical and case management team under the direction of the Case Manager Lead, upholding BRC's core values of Love, Community, Consistency, Honesty, and Hope in every patient interaction. This role requires an active Chemical Dependency Counselor Assistant (CDCA) credential and operates in accordance with Ohio Chemical Dependency Professionals Board (OCDPB) standards, OhioMHAS guidelines, and CARF accreditation requirements.

ESSENTIAL JOB FUNCTIONS

Case Management & Service Planning

  • Develop and implement individualized case management plans in collaboration with the clinical team, including comprehensive assessments, goal setting, service planning, progress monitoring, and support for patient independence.
  • Maintain an active patient caseload at assigned BRC sites, ensuring each patient receives timely, consistent, and coordinated support throughout their engagement with BRC services.
  • Provide comprehensive linkage to community resources to address patients' needs including housing, employment, childcare, education, clothing, food, transportation, financial stability, and other social determinants of health.
  • Serve as a liaison between patients and external agencies, making referrals to mental health, legal, medical, and social service resources as needed to ensure access to appropriate programming and support.
  • Attend patient meetings, court appearances, school meetings, and other appointments as needed to support patient advocacy and service coordination.
  • Support colleagues by assisting with caseloads during absences and collaborating on group case management as directed by the Case Manager Lead.

Clinical Support & Intervention

  • Deliver high-quality substance use, mental health, and supportive intervention services to patients in alignment with BRC's evidence-based treatment model and CDCA scope of practice.
  • Provide prevention, consultation, and educational services to patients and their support systems as directed.
  • Apply trauma-informed, recovery-oriented principles in all patient interactions, recognizing the unique strengths and challenges of each individual served.
  • Collaborate actively with BRC's clinical team — including counselors, nursing staff, and peer support — to ensure integrated, whole-person care for every patient.
  • Stay informed of emerging treatment strategies, best practices, and developments in substance use disorder and behavioral health through ongoing professional development and continuing education required for CDCA renewal.

Documentation & Compliance

  • Maintain accurate, timely, and confidential case records and documentation, including progress notes, service plans, and collateral correspondence, in accordance with BRC policy, OhioMHAS standards, and CARF accreditation requirements.
  • Comply with all applicable confidentiality standards including 42 CFR Part 2 and HIPAA in all patient interactions and documentation.
  • Adhere to all applicable state and local laws, agency policies and procedures, patient rights standards, safety protocols, and the OCDPB Code of Ethics governing CDCA practice.
  • Contribute to BRC's Quality Improvement activities, including peer reviews, OhioMHAS compliance monitoring, and agency quality assurance processes.
  • Maintain active CDCA credential in good standing with the Ohio Chemical Dependency Professionals Board (OCDPB), including all required continuing education hours, throughout employment.

Teamwork & Professional Conduct

  • Actively participate in supervision sessions, case management meetings, treatment team meetings, and other agency-required meetings as scheduled.

  • Maintain professional relationships with patients, staff, and external partners; model appropriate boundaries and ethical conduct at all times.
  • Communicate proactively with the Case Manager Lead regarding caseload concerns, patient safety issues, or operational challenges requiring escalation.
  • Perform other related duties as assigned.

Productivity Requirement

  • Meet and maintain 60% productivity/impact monthly.
