Case Manager - YHO - ILP

Open 31d

Professional Problem Solver. Resource Connector. Hope Builder.

Do you have a knack for helping people figure things out? Can you juggle conversations with schools, families, community partners, and treatment teams—all before lunch? Do you believe every young person deserves a champion in their corner?

If you're nodding along, keep reading.

We're looking for a Case Manager to join our team of difference-makers. This isn't a desk job where every day looks the same. One day you might be helping a teen connect with resources to stay in school, the next you're celebrating a family milestone, coordinating services, or helping someone navigate a tough situation and come out stronger on the other side.

🚀 Your Mission (Should You Choose to Accept It)

Help youth and families build brighter futures by connecting them to resources, supporting their goals, and ensuring they have a team cheering them on every step of the way.

🌟 What You'll Actually Be Doing

  • Becoming a trusted ally for youth and families.
  • Helping clients identify goals and create a roadmap to achieve them.
  • Connecting people with resources they didn't know existed.
  • Partnering with schools, providers, community agencies, and support systems.
  • Turning "I don't know what to do" into "I've got a plan."
  • Supporting clients through challenges, setbacks, and victories.
  • Helping youth build skills for independence, education, employment, and everyday success.
  • Coordinating care so everyone stays focused on what matters most—the client.
  • Managing crises with calm, confidence, and compassion.
  • Creating service plans, documenting progress, and keeping everything moving forward.
  • Celebrating wins—big and small.

🦸 Superpowers We're Looking For

  • You genuinely enjoy helping people.
  • You can build rapport with almost anyone.
  • You stay calm when things get complicated.
  • You're organized enough to keep multiple moving pieces on track.
  • You can think critically, solve problems creatively, and adapt quickly.
  • You understand that sometimes the smallest support can make the biggest difference.

💡 Bonus Points If You Have

  • Mental health case management experience
  • CDCA certification
  • A talent for finding solutions where others see barriers
  • The ability to remember where you parked after a full day in the community
