Case Manager - YHO - ILP
Professional Problem Solver. Resource Connector. Hope Builder.
Do you have a knack for helping people figure things out? Can you juggle conversations with schools, families, community partners, and treatment teams—all before lunch? Do you believe every young person deserves a champion in their corner?
If you're nodding along, keep reading.
We're looking for a Case Manager to join our team of difference-makers. This isn't a desk job where every day looks the same. One day you might be helping a teen connect with resources to stay in school, the next you're celebrating a family milestone, coordinating services, or helping someone navigate a tough situation and come out stronger on the other side.
🚀 Your Mission (Should You Choose to Accept It)
Help youth and families build brighter futures by connecting them to resources, supporting their goals, and ensuring they have a team cheering them on every step of the way.

- Supporting clients through challenges, setbacks, and victories.
- Helping youth build skills for independence, education, employment, and everyday success.
- Coordinating care so everyone stays focused on what matters most—the client.
- Managing crises with calm, confidence, and compassion.
- Creating service plans, documenting progress, and keeping everything moving forward.
- Celebrating wins—big and small.
🦸 Superpowers We're Looking For
- You genuinely enjoy helping people.
- You can build rapport with almost anyone.
- You stay calm when things get complicated.
- You're organized enough to keep multiple moving pieces on track.
- You can think critically, solve problems creatively, and adapt quickly.
- You understand that sometimes the smallest support can make the biggest difference.
