Case Planner coordinates services for children and families in care, maintains case records, and represents the agency in court and school meetings.

The Children's Home of Poughkeepsie has been caring for children throughout the Hudson Valley for over 175 years! We strive to empower children and families to achieve optimal health and well being, and are looking for a Case Planner to work in our Empower program.

Position Summary:

The Empower Case Planner is responsible for the delivery of casework services to families and children in care. The Case Planner is responsible for coordinating services with various agencies, professionals, and providers. The Case Planner represents the Agency at criminal and family court proceedings, school meetings, conferences with LDSS, and any other entity the Agency communicates with. The Case Planner maintains accurate and detailed case records and prepares reports and other documentation required by the agency and/or LDSS.

Essential Functions and Responsibilities:

  1. Fulfills training responsibilities according to Children’s Home, NYS Justice Center, and OCFS regulations.
  2. Is familiar with and adheres to all agency policies and procedures as outlined in the Direct Care Manual, Employee Handbook and Risk Management Manual.
  3. Maintains an individual case work relationship with each youth and family on his/her caseload.
  4. Coordinates and monitors scheduling, transporting, and supervising visitation between youth and their parent(s), sibling(s), and/or approved visiting resource(s).
  5. Represents the Agency in family and criminal court proceedings pertaining to the youth in the program. Coordinates services and communicates with Probation as applicable.
  6. Prepares summaries and reports, maintains current progress notes and documentation, ensures relevant information and significant incidents are incorporated into the case record.
  7. Develops and executes permanency plans with families consistent with the youth’s service plan.
  8. Completes referrals for community support services for youth and their families as needed.
  9. Communicates with the school regarding youth educational progress and needs. Attends academic meetings as needed.
  10. Maintains family contacts through home and agency visits, and provides in home and at the agency casework counseling.
  11. Assists with facilitating and coordinating psychoeducational groups for youth.
  12. Completes Connections documentation within required timeframes including but not limited to progress notes, permanency hearing reports, etc.
  13. Participates in weekly team meetings. quarterly treatment team meetings, Red Flag meetings, and any other meetings pertaining to his/her caseload.
  14. Collaborates with the EMPOWER Behavioral Health Clinician, EMPOWER Aftercare Worker, and Youth Counselors to effectively plan for youth.
  15. Attends all agency and program specific mandatory meetings and training.
  16. Accepts any other tasks delegated by the AED, Child Welfare Programs or required by the program

Schedule: Monday-Friday 8am-5pm, Saturday-Sunday OFF.
