Case Planner
Company Overview
To protect, empower and promote independence for at-risk children and families and those with developmental disabilities through quality community-based services.
Key Responsibilities
- Guide families through a structured, three-phase treatment model: Initial, Baseline, and Stabilization.
- Conduct comprehensive assessments and develop individualized service plans.
- Coordinate multidisciplinary interventions tailored to each family's unique needs.
- Build meaningful relationships with families over an average case duration of 12 months.
- Collaborate with internal and external providers to ensure safety, stability, and well-being.
Requirements
- BSW or bachelor's degree in human services REQUIRED.
- Some knowledge of the child welfare system.
Fluency in English and Spanish a Plus
- Citywide travel required- 14 cases up to 4 visits with each family per month
