Hope For Youth's Residential Services Department provides community based, trauma informed, youth and family driven care to adolescents and young adults in Nassau and Suffolk County. Serving youth from varying points of entry with the child welfare system, juvenile justice system, and runaway and homeless youth/young adults, the opportunities to alter the trajectory for youth in need are abundant. Youth are supported 24/7 by a team of multi-disciplinary professionals, paraprofessionals, and agency administration that consistently meets standards for accreditation and licensing regulations.

Hope For Youth's residential caseworker is a vital team member for the Adolescent Diagnostic and Respite Services program. Supporting youth involved with the child welfare system, juvenile justice system and respite bed network, the caseworker meets youth needs and build strengths utilizing trauma informed Therapeutic Crisis Intervention and motivational interviewing skills. Responsibilities include but are not limited to completing psychosocial assessments as part of comprehensive diagnostic evaluations, gathering documentation, creating youth and family driven service plans, community-based work (home visits), keeping connection to family and resources, supporting youth during crisis, and participating in treatment team conferences. What You'll Be Doing:

  • Function within the agency policy and procedures as outlined in the program practice guide and employee manual.
  • Obtaining necessary documentation to prepare psychosocial assessments and final reviews
  • Preparing case updates and psychosocial assessments; home visits; parent and youth interviews
  • Supervising and documenting of family visits; progress notes
  • Preparing incident reports and notifications, as needed
  • Supporting the overall functioning of the program and residents from a trauma informed perspective
  • Perform other resulting and necessary work to meet the needs and requirements of the clients, the program, Hope For Youth, oversight agency and any other regulatory obligations

Where You'll Spend Your Time:

  • Directly supporting youth in program and community settings

  • Writing/documentation and case management

  • Team meetings, committees, and supervision

  • Ongoing training and professional development

  • Other related work

What We're Looking For:

  • Bachelor's degree in social work or related field

  • Valid NYS Driver's License (ability to safely operate a 7-passenger agency vehicle)

  • Strong writing and time management skills

  • Bilingual candidates encouraged to apply!

  • 1+ year experience working with at-risk youth preferred

  • Flexibility to work varied schedules when needed

Physical Requirements:
Ability to complete reports and logs, sit/stand for long periods, climb stairs, and drive regularly for work.

This opportunity is being offered as Full-Time 35hrs/ week Monday-Friday 9am-5pm with flexible late day.
