CCM and RPM Physician
You will oversee chronic disease management and remote patient monitoring, leveraging AI-driven analytics and blockchain technology to improve patient outcomes. You will conduct virtual patient consultations, collaborate with nurses and care coordinators to implement proactive interventions, ensure documentation and billing compliance for CCM and RPM services, provide telehealth-based patient education, and stay current with digital health trends.
Responsibilities
- Manage and oversee CCM and RPM programs, ensuring compliance with CMS and payer guidelines
- Conduct virtual patient consultations for chronic disease management and remote monitoring
- Utilize AI-driven analytics to assess patient data and optimize treatment plans
- Collaborate with care teams, including nurses and care coordinators, to implement proactive interventions
- Ensure documentation and billing compliance for CCM and RPM services
- Provide telehealth-based patient education on chronic disease management
- Stay up-to-date with digital health trends and contribute to innovative care strategies
Requirements
- MD or DO, Board-Certified in Internal Medicine, Family Medicine, or related field
- Licensed to practice in the United States (multi-state licensure a plus)
- Experience in chronic disease management and telemedicine
- Familiarity with CCM and RPM billing and CMS reimbursement guidelines
- Strong understanding of EHRs, digital health platforms, and AI-driven healthcare analytics
