Join Crest Home Health & Hospice

At Crest Home Health & Hospice, we believe compassionate care goes beyond medical needs. Our Chaplain/Spiritual Counselor provides meaningful spiritual and emotional support to hospice patients and their families during some of life’s most difficult moments.

We are looking for a compassionate, adaptable, and dependable professional who can provide spiritual care while respecting each patient and family’s individual beliefs, values, and traditions.

What You’ll Do

  • Provide spiritual and emotional support to hospice patients and their families based on their individual beliefs and preferences.

  • Assess spiritual needs and participate in interdisciplinary group (IDG) meetings to help develop and coordinate individualized care.

  • Support patients and families during end-of-life care, at the time of death, and throughout the bereavement period.

  • Coordinate with clergy, faith communities, and other community resources when additional spiritual support is needed.

  • Provide or assist with memorial and funeral services when appropriate.

  • Maintain accurate documentation of patient and family visits and services provided.

  • Educate and collaborate with hospice staff, clergy, and community organizations on spiritual needs related to serious illness, loss, and grief.

  • Respond appropriately to urgent or crisis situations and provide support when needed.

What We’re Looking For

  • Graduate of an accredited seminary or school of theology, or appropriate certification in hospital or pastoral ministry.

  • Certificate or degree in pastoral ministry.

  • Two years of experience as a chaplain or spiritual counselor preferred.

  • Recent experience providing direct patient or client contact preferred.

  • Ability to work independently, exercise sound judgment, and respond appropriately in a home-based hospice setting.

  • Strong interpersonal and communication skills with the ability to work respectfully with people from diverse cultural, social, and religious backgrounds.

  • Emotional maturity, flexibility, and the ability to provide compassionate support during difficult and sensitive situations.

  • Current valid driver’s license, auto liability insurance, and reliable transportation.

Why Join Crest Home Health & Hospice?

You’ll have the opportunity to make a meaningful difference for patients and families while working alongside a compassionate interdisciplinary hospice team. If you’re passionate about providing whole-person care and supporting people through the end-of-life journey, we’d love to hear from you.
