POSITION SUMMARY:
The Data Entry & Follow-Up Specialist role focuses on accurately entering and maintaining client data within our e-basis system, ensuring follow-up communication with clients, and collaborating closely with various departments to provide comprehensive support. The ideal candidate will demonstrate a strong commitment to accuracy, confidentiality, and client-centered care. Assist clients in facilitating access to needed services, provides support and encouragement while assisting clients in the recovery process.
RESPONSIBILITIES:

Accurately input and maintain client data and records in the e-basis system, ensuring information is up to date and compliant with organizational standards.

Perform regular follow-up with clients through phone, email, or other communication methods, ensuring all necessary updates and check-ins are conducted in a timely manner.

Collaborate closely with all departments, including counseling, case management, and administrative teams, to ensure smooth communication and the integration of client services.

Generate reports as needed for client progress, attendance, and follow-up activities.

Assist in scheduling appointments, tracking follow-up tasks, and providing reminders for upcoming sessions or deadlines.

Ensure the confidentiality of all client information and adhere to Nexus Recovery Center’s data privacy policies.

Identify any discrepancies or issues in client data and work with appropriate staff to resolve them.

Maintain organized documentation and files related to client follow-up activities, ensuring all interactions are logged and recorded in a timely manner.

Support the team in maintaining a high standard of care and service delivery for clients engaged in recovery programs.

ESSENTIAL DUTIES:

MODEL appropriate and effective coping and self-help recovery skills.
-
PROVIDE strong peer recovery leadership.
-
GUIDE and mentor individuals seeking to achieve or sustain long-term recovery from addiction, regardless of pathway to recovery.

MAINTAIN personal recovery evidenced by remaining 100% abstinent from all mood altering and mind changing substances.

ASSIST clients in developing their own recovery plan and its implementation.
-
PROVIDE case management related functions such as providing referrals, scheduling appointments, shopping, budgeting, using public transportation, and meeting attendance.

PROVIDE transportation as assigned to ensure client access to meetings and community services.

PLAN and implement recovery activities and events.
-
ATTEND court appointments with clients to act as a support/advocate.

ASSIST clients in accessing community services, as indicated.

CONDUCT active outreach efforts to encourage attendance and participation in recovery-oriented, self-help, and pro-social groups.

PROVIDE supportive services during times of client crisis/relapse.
-
TEACH personal care, self-help, and relapse prevention skills to clients.

ATTEND required trainings for the position.

ATTEND monthly All Staff meeting.

PERFORM various clerical tasks, filing, copying, etc.
