About the Role

Select Home Health is seeking a motivated and compassionate Licensed Practical Nurse (LPN) / Account Executive to provide high-quality in-home nursing care while building relationships with healthcare providers and referral sources. This role combines hands-on patient care with sales, community outreach and referral development to support patient well-being and grow our home health services.

Responsibilities

• Clinical Nursing Responsibilities

• Provide skilled nursing care in accordance with physician orders and established care plans.

• Monitor patient conditions, assess pain and educate patients and families on care needs.

• Administer medications safely and follow established medication administration procedures.

• Promote patient safety, confidentiality and quality of care.

• Sales & Referral Development

• Build and maintain relationships with healthcare providers, community partners and referral sources.

• Educate referral partners about Select Home Health services.

• Identify opportunities to expand referral networks and increase patient census.

• Execute outreach plans and track activities in the CRM.

• Represent Select Home Health at community events and networking opportunities.

• Monitor market trends and identify opportunities for growth.

• Administrative & Team Collaboration

• Maintain accurate records and complete required reports.

• Coordinate with clinical teams to support patient needs and referral transitions.

• Collaborate with leadership on outreach initiatives and agency growth.

• Ensure clinical and business development activities comply with applicable regulations and agency policies.

• Communicate patient status and needs with the interdisciplinary care team.

• Complete accurate and timely clinical documentation.

• Participate in required trainings, in-services and meetings.

• Follow agency policies, procedures and emergency protocols.
