Job Summary
Supports the Director of Patient Care Services and clinical team to ensure high-quality patient care, efficient staff coordination, and compliance with all regulatory requirements in a home health setting.
Key Responsibilities
Coordinate clinician schedules and patient assignments to maximize efficiency and coverage
Monitor productivity and ensure clinicians meet visit expectations
Support onboarding, training, and ongoing staff development
Promote strong interdisciplinary communication and teamwork
Ensure compliance with state, federal, and Medicare regulations
Conduct chart audits and support quality assurance initiatives
Communicate with physicians, patients, families, and pharmacies
Assist field staff with orders, lab follow-ups, and care coordination
Maintain accurate patient and medication records
Qualifications
Required:
Active clinical license
4+ years of clinical experience (home health preferred)
Knowledge of healthcare regulations and Microsoft Office
Strong organizational, communication, and decision-making skill
Bachelor’s degree in a clinical field
2+ years of supervisory experience
