Clinical Navigator
Position Summary:
The Clinical Navigator will function as a team member in coordinating services for children and families under the care and supervision of the Assistant Director. The Navigator will be available to families and children to evaluate their needs and connect them to other services and supports in the community.
Role and Responsibilities
This person’s job responsibilities include but are not limited to the following:
- Receive all referrals for clinical services made to the agency
- Schedule and conduct an initial intake on all referrals to assess child and family’s strengths, needs, abilities and preferences
- Prepare and implement individualized wellness plan and recommendations for the child and family
- Implement a professional development plan on managing the impacts of trauma
- Recognize and assess symptoms of trauma to prioritize needs and provide support and crisis management services and counseling if needed
- Maintain weekly schedule to include visits, in-home and at the center
- Ability to support the Center’s culture, growth, and success through communication, accountability, and positivity
- Provide on-going support and information to families that conform to regulatory and professional standards in congruence with the Center’s mission, vision, and values
- Mindfully integrate the Trauma Informed Care Model and Person-Centered Approach into all procedures, practices, and daily interactions
- Conduct support groups as scheduled by the Center
- Maintain an awareness of services available throughout the community and assist families served in accessing appropriate resources
- Maintain timely case notes, file documentation and required paperwork per Center policy
- Maintain productivity standards and requirements and track progress and outcomes
- Communicate with school, psychiatrist, therapist, pediatrician, and other professionals working with the child and family if needed
- Attend individual supervision, staff meetings and trainings
- Follow Florida Child Abuse Reporting Rules
- Share all safety-related concerns with direct supervisor and the Director of Operations and Clinical Services
- Perform all other duties as requested or assigned
