Description

Clinical Nurse Consultant (CNC) Coordinator – Older Person Care Hub (OPCH)

  • Part Time fixed term | 48 hours per fortnight
  • This nurse-led, community-focused role is ideal for an experienced clinician
  • Join a values leg organisation

Join the Royal Melbourne Hospital’s innovative Older Person Care Hub (OPCH), providing a home-based alternative to Emergency Department and hospital presentations for older adults, initially focusing on people who have experienced a fall.

As the Clinical Nurse Consultant Coordinator, you’ll lead referral triage and service coordination, ensuring timely face-to-face assessments within 24 hours and working closely with geriatricians and allied health staff to develop and monitor individualised care plans. You’ll oversee the four-week care pathway, coordinate follow-up and support safe transitions, helping older people remain safely at home and reducing avoidable hospital admissions.

The service currently operates five days a week and is planned to expand to seven days in 2027. The successful applicant will be expected to work approximately every third weekend as part of this expansion.

What you’ll be doing

  • Receive and triage referrals from the Virtual Victorian Emergency Department (VVED), RMH Geriatrician in ED (EDGE), and other partners as the service expands (e.g. Ambulance Victoria, GPs).
  • Manage the day-to-day running of the Hub
  • Collaborate with geriatricians, CNC and allied health teams to develop and implement individualised care plans.
  • Coordinate timely face-to-face assessments within 24 hours of referral and ensure appropriate follow-up via telephone or telehealth.
  • Oversee a structured care pathway
  • Maintain effective communication with key stakeholders, including families, GPs, and other health services, using telehealth, EMR, and written summaries.

About you

You are a Registered Nurse with at least five years of post-registration acute experience, strong clinical assessment skills and the ability to work autonomously in a community-focused setting. You bring sound clinical judgement, a collaborative approach and a commitment to supporting older people to remain safely at home.

You will bring:

  • Current registration as a Registered Nurse with AHPRA.
  • Highly developed clinical assessment, reasoning, problem-solving and prioritisation skills.
  • Strong written and verbal communication skills, with the ability to maintain accurate clinical documentation.
  • The ability to work independently and collaboratively, manage competing priorities and adapt to changing needs.
  • Experience building effective relationships with multidisciplinary teams and key stakeholders.
  • An understanding of chronic disease management and community care, ideally with experience in acute geriatric or acute community care.
