We are seeking a Clinical Pharmacy Manager to join our team! The Clinical Pharmacy Manager serves as the clinical and operational leader of Community Clinic's ambulatory clinical pharmacy team. This is a working clinical leadership role: the manager leads the team's clinical practice standards, program operations, quality measure performance, and integration with primary care, specialty care, and external pharmacy partners, while also managing a reduced patient panel. The schedule for this position full-time, with a schedule Monday - Friday 8:00AM - 5:00 PM

Essential job functions

1. Clinical Practice - Performs all duties of a clinical ambulatory care pharmacist, including:

  • Conducts patient consultations under signed Collaborative Practice Agreements (CPA) with supervising primary care physicians — initiating, titrating, modifying, and discontinuing medications; ordering laboratory studies; documenting clinical decisions in the EHR per CPA protocol.
  • Provides Comprehensive Medication Reviews (CMRs) for high-risk patients; generates Personal Medication Action Plans; communicates summaries to primary care providers.
  • Conducts, or delegates, adherence barrier review for high-risk patients and regimen simplification for patients with Proportions of Days Covered (PDC) <0.80.
  • Performs regular review of Remote Physiological Monitoring (RPM) device data and initiates CPA-driven clinical response under physician supervision.
  • Delivers cross-site tele-pharmacy visits, providing clinical coverage for physical sites via telehealth infrastructure.
  • Maintains scope of practice across all applicable disease states.
  • Documents PharmD clinical time toward the per-code minimums for Chronic Care Management (CCM), Advanced Primary Care Management (APCM), and Remote Physiological Monitoring (RPM), supporting physician billing under documented supervision.

2. Clinical Team Leadership - Provides clinical and administrative leadership for the ambulatory clinical pharmacy team. Specifically:

  • Holds direct supervisory authority for clinical pharmacists, including performance management, professional development, schedule coordination, and clinical practice review.
  • Provides oversight of Pharmacy Technician team.
  • Coordinates with Director — in partnership with HR — on interviewing, hiring, firing, coaching, counseling, training, and other employee relations matters in relation to Clinical Pharmacy program.
  • Collaborates with Director of Population Health and other leadership on shared workflows.
  • Conducts weekly team huddles, monthly case conferences, and annual performance reviews aligned with clinical and quality objectives.
  • Mentors the team on clinical practice standards, CPA workflow, CMR delivery, and documentation; identifies and facilitates applicable Board Certification opportunities.

3. Program Operations – Collaborates to direct performance of clinical pharmacy services, including:

  • Monitors patients' PDC: adherence gap identification, intervention assignment, and outcome tracking.
  • Collaborates with Value-Based Care team to drive performance for HEDIS/PQA medication adherence measure.
  • Confirms patient prescription routing to Community Clinic 340B-registered contract pharmacies; investigates and resolves routing gaps; produces monthly organizational 340B benefit reports for leadership.
  • Engages cross-functional teams to maintain accurate documentation flow for physician-billed care management codes.
  • Ensures program compliance with all relevant legal regulations.

4. External Coordination and Strategic Development - Represents Community Clinic's clinical pharmacy program in external relationships and contributes to program strategic growth:

  • Coordinates with supervising primary care physicians on CPA scope, protocol updates, and case-specific clinical decisions; serves as primary clinical liaison between the pharmacy team and the medical staff.
  • Builds and maintains relationships with Community Clinic 340B contract pharmacies; coordinates data-sharing agreements for the PDC pipeline; pursues partnerships with other relevant pharmacy networks as opportunities arise.
  • Interfaces with PASSE, Medicare Advantage, and other payer quality teams on HEDIS/PQA performance reporting, gap closure activity, and quality bonus attribution.
  • Contributes to value-based contract negotiation and renegotiation by providing outcomes data, clinical pharmacy performance metrics, and total-cost-of-care impact analysis, in collaboration with the Director.
  • Represents Community Clinic in professional organizations and local/regional forums as needed.
  • Identifies and pursues additional grant funding, Notice of Funding Opportunities, and value-based contract enhancements to support program sustainability.
