About the Opportunity

Care Finders Total Care, a Modivcare Personal Care Service, is seeking a Per Diem RN Clinical Supervisor to support patients and home health caregivers throughout Mercer and North Burlington Counties, NJ.
This field-based opportunity is ideal for a Registered Nurse who enjoys working independently, providing clinical oversight, and helping patients receive safe, high-quality care in the comfort of their homes.
In this role, you'll conduct nursing assessments, perform supervisory visits, provide clinical guidance to caregivers, and collaborate with the care team to ensure patients receive services tailored to their individual needs.

What You'll Do

• Conduct initial nursing assessments, reassessments, and supervisory visits in patients' homes.
• Supervise, evaluate, and provide clinical guidance to Certified Home Health Aides (CHHAs).
• Provide clinical orientation, education, and training to caregivers.
• Develop, review, and update individualized plans of care in collaboration with physicians and the care team.
• Monitor patients' conditions and communicate changes to the appropriate care team members.
• Educate patients and caregivers regarding nursing care and related needs.
• Coordinate services to support continuity of care.
• Maintain accurate and timely clinical documentation in accordance with company policies and applicable regulations.

What We're Looking For

Required Qualifications

• Active New Jersey Registered Nurse (RN) license or appropriate authorization to practice nursing in the state.
• Current CPR certification.
• Strong clinical assessment, communication, and patient education skills.
• Ability to work independently in a field-based home care environment.
• Ability to travel throughout Mercer and North Burlington Counties for assigned patient visits.

Preferred Qualifications

• Previous nursing experience in home care, home health, or community-based nursing.
• Experience conducting patient assessments, care coordination, or supervisory visits.
• Bilingual proficiency in English and Spanish.

Schedule & Service Area

• Employment Type: Per Diem | Field-Based
• Territory: Mercer and North Burlington Counties, NJ
• Schedule: Approximately 1–2 days per week, with visits coordinated when the patient's assigned home health aide is present.
• Travel: Throughout assigned patient service areas.

Visit assignments are based on patient needs and operational demand. Per diem hours and visit volume may vary.

Per Diem Compensation

Earn $55–$70 per visit, depending on visit type, plus mileage reimbursement.

• $55 – Monitoring Visits / Office Hours
• $65 – Reassessment Visits

Mileage Reimbursement: Provided at the current federal rate for eligible travel.
