The In-Person Clinician provides direct, community-based behavioral health services to individuals enrolled in Community Stabilization. This role requires clinical expertise, strong collaboration skills, and the ability to work effectively within a multidisciplinary team in high-acuity environments.
Key Responsibilities:
- Provide a minimum of 12 hours per week of direct, face-to-face community-based services, delivered in the client’s natural environment in collaboration with a QMHP.
- Conduct comprehensive clinical assessments to determine service needs, risk factors, and stabilization planning.
- Develop and implement individualized stabilization plans in coordination with clients, case managers, and crisis staff.
- Facilitate care coordination across providers, including internal clinical teams and external community partners.
- Maintain consistent communication with staff to ensure service quality, compliance, and accurate documentation in accordance with agency and regulatory standards.
- Complete timely, thorough, and clinically sound documentation that reflects services rendered and client progress.
- Initiate referrals and coordinate linkages to additional behavioral health, medical, housing, or community resources as clinically indicated.
- Collaborate with interdisciplinary professionals to develop integrated diagnoses and treatment approaches that support long-term stabilization and recovery.

Education:
- Master's (Required)
Experience:
- Clinical: 2 years (Required)
License/Certification:
- LMHP or LMHP-S (Required)
