Community-Based Case Manager (6924)

Position Overview

The Community Based Case Manager is responsible for:

• Developing and implementing Individual Service Plans (ISP).
• Supporting community inclusion and independent living skills.
• Providing care coordination, direct care, and supervision as needed—including personal care and daily living activities.

PRIMARY DUTIES:

• Coordinate care and provide support in:
  • Education and social skills
  • Meal planning and grocery shopping
  • Personal care and hygiene
  • Clothing care and travel training
  • Money management and housing
  • Community participation and religious practices (if desired)
• Transport individuals to medical appointments and:
  • Follow up on medical care plans
  • Educate individuals, families, and staff on self-directed care
• Support healthy, culturally appropriate meal planning and purchasing
  • Provide transportation and skill-building in grocery shopping
