Community Care Assistants

Job Description:

• Establish and implement Individual Care Plan (ICP) for clients, including appropriate intervention plans.
• Assist in identifying clients who may show early onset of medical conditions.
• Conduct and facilitate group and individual activities, ensuring holistic care for clients.
• Guide and assist clients in daily activities such as exercises, games, handicrafts and outings.
• Assist with transportation and escort clients to, from and within the centre.
• Serve meals and provide support with personal hygiene care.
• Measure, record, and monitor clients' vital signs, including blood pressure, heart rate, pulse, and respiratory rate.

Requirements:

• Prior experience in eldercare, healthcare, or social service sector preferred.
• Able to perform basic caregiving tasks, including personal hygiene care and meal assistance.
• Good communication and interpersonal skills to interact effectively with clients, caregivers, and colleagues.
• Bilingual and able to converse in dialects preferred.
• Patient, compassionate, and committed to enhancing clients’ well-being.
• Physically fit to assist clients in mobility and daily activities.
