Community Care Liaison - Healthcare Sales & Relationship Development

Butte Home Health & Hospice is seeking a relationship-driven Community Educator to build referral partnerships and increase access to Home Health, Hospice, Palliative Care, and other supportive services.

This field-based role works with hospitals, physician offices, skilled nursing facilities, senior living communities, and other healthcare partners throughout Butte and surrounding counties. Responsibilities include developing new accounts, maintaining existing referral relationships, providing education and presentations, identifying growth opportunities, tracking outreach activity, and collaborating with internal teams to support a positive referral experience.
