Adult Behavioral Health Case Manager (Vulnerable Adults / Geriatric Focus)

Are you passionate about advocating for vulnerable individuals and helping adults navigate mental health, physical health, and community resources? We are seeking a dedicated Adult Behavioral Health Case Manager to join our team in Newton, KS.

In this role, you will work closely with adults, older adults, caregivers, and referral partners—including the Department for Children and Families (DCF)—to reduce vulnerability, build natural supports, and enhance overall quality of life.

Position Overview

Using the Strengths Model of case management, you will provide community-based support, outreach, and advocacy for individuals who may be experiencing mental health challenges, physical health needs, or risks related to abuse, neglect, or exploitation. You will work within an interdisciplinary team (including participation in daily ACT team meetings) to link clients to vital housing, financial, medical, and daily living resources.

Key Responsibilities

  • Community Outreach & Advocacy: Establish rapport with new patients through assertive outreach. Serve as a key advocate to connect clients with housing, financial support, medical care, and community resources.

  • Person-Centered Case Management: Partner with clients and caregivers to assess strengths, set goals, and teach daily living, self-management, and symptom-coping skills.

  • Interdisciplinary Collaboration: Actively participate in daily ACT team meetings, treatment planning, and case consultations with internal clinical staff and external partner agencies (such as DCF).

  • Crisis Assistance & Group Support: Provide immediate crisis intervention support as needed and assist in co-leading psychosocial support groups.

  • Health & Wellness Support: Assist clients in understanding and managing physical health conditions, promoting self-direction and overall health literacy.

  • Documentation & Compliance: Maintain timely and accurate clinical records (progress notes completed within 72 hours) and complete required state reporting (e.g., AIMS).
