Case Manager (Children & Family Services)
Pay: $24.15/hour + Monthly Bonus Potential
Schedule: Monday – Friday (Day Shift with Flexible Scheduling)
Location: McPherson, KS (On-Site & Local Community Travel)
Are you passionate about making a real, lasting impact in the lives of youth and families? Prairie View is seeking a dedicated Case Manager to provide strength-based, therapeutic support and care coordination to children, adolescents, and their caregivers across home, school, and community settings in McPherson.
In this role, you’ll work directly with youth and families to build life skills, de-escalate crises, link them to critical community resources, and advocate for their success—all within a supportive, team-oriented environment.
What You’ll Do

Direct Client & Family Support: Deliver community-based case management interventions in homes, schools, and local settings focused on problem-solving, goal-setting, life skills, and healthy routines.

Crisis De-escalation & Safety: Provide effective crisis intervention, establish safe boundaries, and utilize trauma-informed care tailored to child and adolescent needs.

Resource Advocacy: Link families to essential community services, teach parenting/life skills, and advocate for their rights across various support systems.

Interdisciplinary Collaboration: Work closely with QMHP clinicians, treatment teams, prescribers, and external referral sources to coordinate comprehensive care plans.

Program Involvement: Support children in specialized care settings, including Prairie View’s Summer Intensive Rehabilitation Program (SIRP) and psychosocial skills groups.

Administrative & Clinical Documentation: Maintain timely progress notes and clinical records in compliance with state and agency standards (progress notes within 48 hours).
