Job Summary:
Join our team as a Service Facilitator and Care Coordinator for youth and young adults enrolled in the PROPs First Episode Psychosis program. PROPs provides services primarily under the Dane County Comprehensive Community Services (CCS) system of care, which offers an array of mental health and substance use services in a team of providers of the participant’s choosing.

Duties/Responsibilities:
- Coordinate care for 10-15 youth and young adults experiencing their first episode of psychosis.
- Complete intake process, interviewing consumer and family members to build the comprehensive assessment, recovery plan, and provider team to support the individual.
- Coordinate and facilitate consumer Recovery Team Meetings at a minimum of every six months to update recovery plan and continually assess treatment goals.
- Utilize effective strategies to engage program participants, family members, and natural supports to enhance motivation and encourage meaningful, collaborative, and strengths-based goal planning and progress.
- Maintain regular contact with program participants to a degree sufficient to support effective engagement to assist with making and evaluating progress.
- Provide or obtain support and crisis/emergency services for consumers as appropriate.
- Monitor the provision and quality of services provided and act as the liaison when new services/resources need to be sought or developed.
