As a Health Advocate, Float, you are an adaptable, highly skilled problem-solver who ensures continuity of care across our patient panels. Your primary goal is to cover the most critical parts of our business, stepping in to complete visits by being available when regular team members are unavailable or out of office.

At variable FTE based on business needs, you will serve as a core capacity anchor by providing reliable, part-time coverage to absorb temporary staffing gaps, manage high patient volumes, and handle urgent needs across our clinical pods. Rather than owning a dedicated patient panel of your own, you will step into established, permanent panels on a temporary basis when called upon—partnering closely with our longitudinal care teams and acting as their dedicated coverage partner during those periods. You will deliver excellent in-person and virtual care for initial onboarding visits, routine follow-ups, and urgent social care needs. This role requires a nimble, tech-fluent team member who can seamlessly drop into varying Health Advocate panels, deliver high-quality care to complex patients, and execute tight handoffs back to the care team.

You'll visit patients in their homes to facilitate video visits, capture vitals, follow up on critical social needs, and support complex care coordination. By covering these panels as needed, you will prevent gaps in care, avoid unnecessary hospitalizations, and ensure our patients always have the support they need to stay safe, stable, and thriving at home. Because you'll move quickly between panels, you'll rely heavily on our technology—EHR systems, video platforms, mobile devices, and AI-enabled tools—to get up to speed fast and deliver seamless care.

Responsibilities will include:

Coverage & Continuity of Care:

Act as a flexible resource to cover the most critical parts of the business, completing in-home and virtual visits by being available when assigned team members are out of office or unavailable
Clinical Support & Vitals Capture:
-
Independently capture vitals using connected devices and digital tools, track changes in condition, observe the home environment, and communicate early warning signs to the care team
Critical Social Needs Follow-up:

Identify, document, and aggressively follow up on critical social needs (e.g., food insecurity, housing instability, transportation issues, safety concerns) to ensure continuity in resource navigation and benefits assistance
Care Coordination Needs:

Support essential care coordination needs, including patient accountability with medication changes, follow-up on referrals, appointment scheduling, testing, and home health services
Facilitate Virtual Visits:

Set up, support, virtually troubleshoot, and conduct virtual visits with primary care or behavioral health clinicians, confidently navigating video platforms and connected devices to ensure smooth technology use for both patients and clinicians
Reinforce Care Plans:

Review and quickly understand patient care plans—often with the help of our digital and AI-assisted tools—to effectively coach patients and hold them accountable to self-management goals during coverage periods
Document and Communicate Effectively:

Maintain highly accurate EHR documentation, leveraging available digital and AI-supported workflows to ensure seamless handoffs and consistent updates back to the primary care team after float coverage

Required

Experience in a direct patient care, home-based role, or float pool setting such as Medical Assistant, Health Coach, Community Health Worker, EMT or similar

Strong interpersonal and communication skills, with a unique ability to quickly build rapport and trust with patients you are meeting for the first time

Comfortable in managing emergency situations, as they arise, to ensure the safety and health of medically and socially complex adults

Competence in basic clinical tasks (vital signs, observation, infection control) and technology (smart devices, video platforms, EHR documentation), with the ability to learn and adopt new systems quickly

Valid unrestricted driver's license and access to an insured vehicle for daily use. The assigned geographic area may include locations requiring variable drive times

Preferred

Medical Assistant (CMA) or Community Health Worker (CHW) certification, or 2+ years experience in a healthcare setting

Experience with value-based care, home-based care, or virtual primary care models

Experience using AI-powered or AI-assisted technology (e.g., AI scribes, documentation tools, clinical decision-support tools, or other digital health applications) in a professional setting

Bilingual or multilingual fluency in Spanish or Arabic is a strong plus; additional fluency in other commonly spoken U.S. languages are also a plus
