Summary
Connects underserved individuals and families to healthcare, food assistance, housing, and public benefits while coordinating community wellness programs and outreach events in Hampton Roads.
- Assist individuals and families in identifying and accessing healthcare, behavioral health, nutrition, housing, employment, transportation, and other essential community resources.
- Conduct comprehensive needs assessments to identify barriers impacting health, stability, and self-sufficiency.
- Provide resource referrals and follow-up support to ensure successful connections to services.
- Serve as a trusted liaison between residents and community service providers.
- Support community members in navigating complex eligibility requirements for federal, state, and local assistance programs.
- Assist residents in understanding and accessing available healthcare coverage, including Medicaid, Medicare, Marketplace plans, and other healthcare programs.
- Educate community members regarding policy changes that may affect eligibility for healthcare and public benefits programs.
- Conduct outreach and enrollment support activities for qualified health and assistance programs.
- Help individuals access primary care, preventive services, specialty care, pharmacy assistance, and behavioral health resources.
- Collaborate with healthcare partners to improve care coordination and reduce barriers to care.
- Promote healthy lifestyle behaviors through education, engagement, and community-based wellness initiatives.
- Coordinate and support participation in healthy living reward programs, wellness challenges, fitness activities, nutrition education, and preventive health screenings.
- Encourage participation in evidence-based programs designed to improve health outcomes and reduce chronic disease risk factors.
- Track participant engagement and health improvement outcomes associated with wellness initiatives.
- Facilitate workshops and educational events related to nutrition, physical activity, chronic disease prevention, and overall wellness.
- Coordinate community food distribution events, food pantry operations, and mobile food outreach activities.
- Assess food insecurity needs and connect households with food assistance resources.
- Collaborate with food banks, grocery partners, farmers, community gardens, and hunger relief organizations.
- Assist with enrollment in nutrition assistance programs and community-based food support initiatives.
- Maintain accurate inventory, participant records, distribution logs, and program reports as required.
- Support special food distribution initiatives during emergencies, extreme weather events, and community crises.
- Develop and implement outreach strategies targeting underserved and vulnerable populations.
- Represent HRCAP at health fairs, food distributions, community events, neighborhood meetings, and stakeholder gatherings.
- Build relationships with healthcare systems, community organizations, schools, employers, faith-based organizations, and local government agencies.
- Facilitate public awareness campaigns focused on health equity, food access, preventive care, and self-sufficiency.
- Recruit participants and volunteers for community health initiatives and outreach activities.
- Maintain regular contact with participants to monitor progress and address ongoing needs.
- Track referrals and assess outcomes to ensure individuals receive needed services.
- Coordinate services with internal HRCAP programs including Family-Centered Coaching, Emergency Services, Senior Services, Veteran Services, Housing Programs, Energy Assistance, and Workforce Development programs.
- Advocate on behalf of participants to help overcome barriers to service access.
- Maintain accurate case records, outreach documentation, participant files, and service data.
- Enter data into agency databases and reporting systems in a timely and accurate manner.
- Monitor performance outcomes and assist with grant reporting requirements.
- Collect participant success stories and community impact data.
- Ensure compliance with federal, state, local, and agency policies governing program operations and confidentiality.
- Develop and maintain strategic partnerships with healthcare providers, hospitals, managed care organizations, food banks, social service agencies, and community organizations.
- Participate in local coalitions and collaborative efforts addressing community health needs and food security.
- Assist in identifying funding opportunities and community initiatives that support health and wellness goals.
- Promote integrated service delivery models that improve outcomes for families and communities.
- Perform other duties as assigned by supervisor.
- Represent the organization in professional, civic, and community groups as requested by the CEO.
- Helps establish performance measures and monitors outputs and outcomes.
- Comprehensive understanding of data systems, including tracking, reporting, monitoring, customer satisfaction, and evaluation.
- Ability to work independently, take initiative and be an effective team member.
- High-level communication and interpersonal skills are a must.
- Exceptional time management skills with ability to multitask; ability to work well under pressure, prioritize workload, and meet tight deadlines.
Requirements
- Obtain membership with the Virginia Community Health Work Association within six months of employment.
- Obtain Community Health Worker certification within 90 working days of employment.
- Bachelor's degree in human services, social services, sociology, psychology,
- Experience with program eligibility evaluation.
- Must possess some experience working with TANF and VIEW eligible clients.
- Experience in MS Office, Excel, PowerPoint, and Word.
- Criminal record check, driving record, and credit report required for employment
- Track record for maintaining and deepening relationships with clients, community partners, etc.
- Adept at networking, building relationships, and community engagement
- Ability to work across the organization to deliver solutions that meet internal and external objectives
