Job Title: Community Health Worker/Case Manager, Veteran Community Center (VCC)
Location: 1060 Howard Street, San Francisco, CA 94103
Schedule: Monday through Friday, 11 am – 7 pm
Reporting Type: In-person
Employment Type: Non-Exempt AND Full-Time
Salary Range: $35.33 per hour
Summary:
The Community Health Worker/Case Manager provides trauma-informed, person-centered support to individuals experiencing barriers related to housing instability, mental health, substance use, medical needs, and access to community resources. This position serves as a bridge between clients, healthcare providers, and community-based services by offering advocacy, care coordination, resource navigation, crisis intervention, and ongoing case management support.
The Community Health Worker/Case Manager works collaboratively with interdisciplinary teams to promote client wellness, stability, self-sufficiency, and engagement in services while fostering a safe and supportive environment.
Responsibilities:
Client Services:
- Actively engage clients by reviewing their enrollment packets with them, conducting intakes, and engaging regularly in case management services.
· Coordinate referrals to housing, behavioral health, substance use treatment, benefits, employment, and other supportive services.
- Assist clients in filling out applications for public benefits, identification documents, housing programs, and healthcare services.
- Monitor client progress and maintain regular follow-up appointments.
- Advocate for clients with external providers, agencies, and systems of care.
- Maintain accurate and timely case notes, documentation, and data entry in accordance with agency standards.
- Ensure clients understand the organization’s behavior policies and support them in following the policies so that they may continue to access services.
- Develop and implement a schedule of groups, and other events that align with client needs and organizational goals in collaboration with internal and external entities.
- Develop strategies to increase veteran engagement and program participation to meet goals and outcomes of all funding sources.
Community Health & Care Coordination
- Educate clients on health-related topics including wellness, preventive care, mental health, harm reduction, and available community resources.
- Support coordination of medical and behavioral health appointments.
- Assist clients in navigating healthcare systems and accessing community-based care.
- Collaborate with healthcare providers and multidisciplinary teams to ensure continuity of care.
Grants and Contracts:
- Adhere to program budgets set by the Program Manager or other leadership.
- Review grant and program deliverables with leadership to help the program implement goals, developing processes and procedures to meet grant deliverables.
- Work within the context of active MOUs with external partners and stay in compliance.
- Understand reporting requirements and ensure timely submission of program data and reports for various funders.
- Preparing for and ensuring success of all grant and contract audits.
- Responsible for timely reports and ad-hoc presentations as requested by leadership.
Crisis Intervention & De-escalation:
- Respond appropriately to client crises using de-escalation and conflict resolution techniques.
- Follow agency protocols regarding safety concerns, incident reporting, and emergency response procedures.
- Maintain professional boundaries while supporting clients through challenging situations.
Administrative & Program Support:
- Maintain confidential and organized client records.
- Participate in team meetings, training, case conferences, and supervision.
- Assist with program operations, data collection, and reporting requirements.
- Perform other duties as assigned.
Supervisory Responsibilities (If Applicable)
· Temporarily act as team lead in the absence of the Program Manager and Community Case Manager.
Desired Qualifications:
Experience:
- A minimum of 1–2 years’ experience working with underserved or vulnerable populations is required.
- Fluency in English and ability to effectively read, write, and communicate in English with staff, clients, and external partners and stakeholders.
- A minimum of 1 year of experience in case management, outreach, behavioral health, homeless services, substance use disorders, or social services is required.
- Knowledge of community resources, healthcare systems, and social service programs (i.e. Medi-Cal, SSI, CalFresh, VA) is required.
- Strong interpersonal, communication, and organizational skills are required.
- Ability to work effectively with individuals from diverse backgrounds is required.
- Basic computer skills including documentation, data entry, and the ability to accurately fill out a timecard is required.
- Bilingual/multilingual (Preferred)
Education:
- Community Health Worker Certification, or Peer Support Certification (Preferred)
- Associate’s degree (or higher) in Social Work, Human Services, Psychology, Public Health, or related field (Preferred)
- CPR/First Aid Certification (Preferred)
