As a Pediatric Community Health Worker, you'll be a trusted advocate, connector, and ambassador for COPA, helping children and families navigate healthcare, access community resources, and overcome barriers that impact their health and well-being. What You'll Do

  • Screen patients and families for social determinants of health and connect them with community resources, healthcare services, and support programs.
  • Serve as COPA's representative in the community by building relationships with local organizations, schools, and community partners to strengthen access to care.
  • Independently manage referrals, patient follow-up, and family navigation while exercising sound judgment and initiative to remove barriers to care.
  • Educate and advocate for patients and families by coordinating services, providing culturally responsive support, and helping families successfully navigate healthcare and social service systems.
  • Document patient interactions, monitor outcomes, and collaborate with providers and care teams to improve health equity and deliver exceptional, family-centered care.
  • Assist patients and families with Oregon Health Plan (OHP) enrollment and insurance applications by providing education, completing applications, managing follow-up, documenting activities, and serving as a liaison between patients and state agencies to ensure timely access to coverage.
