Community Outreach Worker (COW)
Job Summary:
The Community Outreach Worker (COW) performs community outreach in response to the health care access needs of Holmes, Madison, Leflore, and surrounding communities. Serves as a central resource for community questions and referrals for both consumers and providers. Builds relationships with community resources, public health, schools, faith community, and other social service organizations to identify and develop resources and remove barriers to accessing health and social services.
Essential Duties and Responsibilities:

Review various reports to identify gaps in care as it relates to pap smears, mammograms, colonoscopies, etc.

Utilize population health systems such as Athena, Aledade, WellCare, and Magnolia portals to outreach patients for annual wellness visits, transition of care, follow-up emergency room visits, export documentation required for Huddle sessions, appointment follow-up for patients, and outreach for High Risk COVID patients.

Communicate and collaborate professionally and effectively with medical providers, medical support staff, front desk staff, case management, medical records, and administration.

Responsible for the strict confidentiality of Medical Records.

Establish and maintain a list of service organizations and resources for client referral.

Inform and assist homeless individuals with access to services and associated community resources, health, dental, mental health, and basic needs.

Connect clients or consumers to information on publicly sponsored health insurance.

Maintain statistics/data on contacts; identify needs, problems, and service gaps.

Attend appropriate community or networking meetings to facilitate outreach and gathering of information to support consumer access to healthcare.

Work with local schools, faith community, senior centers, law enforcement, community partners, and employers to build awareness of Mallory CHC’s services.

Develop outreach materials; Mallory CHC educational flyers/handouts, provide info for Mallory website, Facebook, Twitter postings to build awareness of Mallory CHC’s services, activities, and events.

Serve as liaison between Mallory CHC and the community.

Attend school functions/meetings to educate faculty, parent/guardians, and students about services offered.

Gather information regarding health care needs of students, recommend/implement outreach strategies to increase student access to healthcare.

Attend designated meetings as an employee and medical staff member.

Perform other necessary duties as required by the Community Health Center to meet the goals of providing primary health care.
