The Opportunity
You’ll manage a large panel of Medicare patients with chronic conditions, delivering monthly clinical calls that drive real outcomes and reduce hospitalizations.
This role is built for nurses who can operate independently, manage complexity, and execute consistently at a high level.
We expect our Care Coaches to treat this role as their primary professional focus, not a side gig.

What You’ll Do
- Run monthly clinical calls with chronic care patients
- Drive behavior change through coaching and education
- Maintain precise, compliant documentation using our platform
- Close preventive care gaps and coordinate services
- Update care plans and track interventions
- Support transitions of care to reduce readmissions
- Manage your panel efficiently and consistently

Compensation — Built for Performers
Your earnings scale with your output.
- $15 per completed clinical encounter per patient
- Large caseloads = higher income
- High performers can exceed $3,000/month
- $300 performance bonus after 3 months of consistent results
- No cap on productivity

- Flexible scheduling between 8am–5pm EST, Monday–Sunday
- Fully remote
- 1099 contractor role

Requirements
- Current unrestricted Compact RN license
- 3+ years clinical experience
- Strong internet and dedicated workspace
- Confident with multiple software platforms
- Strong communication and critical thinking skills
Strongly preferred:
- Case management or chronic disease management
- EHR experience
- Motivational interviewing
- Diabetes education

- Are you comfortable with pay timing being monthly, 40 days after the last day of the month for your first paycheck? Thereafter, your pay will be about 30 days after the last day of the month.
- Are you comfortable working as a 1099 Independent Contractor? (responsible for own taxes, computer, and insurance). CLHealth does not provide equipment, reimburse for internet access, or pay for malpractice insurance.
- Are you able to manage a panel of approximately 70 Medicare patients and complete required monthly clinical calls for each patient?
- Can you consistently commit to working a minimum of 25 hours per week across 3–4 weekdays during standard business hours (9am to 6pm ET) in order to manage a caseload of approximately 70 patients and complete at least 95% of required patient calls? Please consider any current employment or scheduling obligations before answering.
- Do you have at least 3 years of full-time experience working as an RN with adult or Medicare patients with multiple chronic conditions (e.g., CHF, COPD, diabetes) and is this experience clearly documented on your resume? Please include only experience in an RN role working with adult patients, and do not include experience in other roles or with non-adult populations.
- This position is compensated at $15.00 per completed 20-minute clinical patient visit (not hourly pay). The pay rate is non-negotiable. Are you willing to work under this compensation structure? If you are not fully in agreement with the pay, please do not apply.
- Are you comfortable working independently in a remote setting with productivity and communication expectations (response time within 24–48 hours)?
