Overall Function:
Assesses service needs and provides assistance in linking and accessing health and social service systems and interventions as identified through a life domain appraisal.
Essential Functions:
- Acts as the individual/family’s single point of contact for multiple health and social service system linkages, navigation, and referral to address barriers to substance use treatment services and recovery.
- Links individuals/families to supports and recovery-focused services.
- Identifies treatment and recovery barriers and ‘walks with the individual’ to navigate through the identified barriers.
- Monitors the individual/family’s participation in services and provides support to encourage participation.
- Assists in addressing social determinants of health, which may be a barrier to treatment and recovery.
- Coordinates and maintains contacts and visits with referral sources and represents the program and agency at various training sessions, community education presentations, and meetings.
- As necessary, participates in all appropriate staff meetings and maintains beneficial dialogue with all members of the community referral sources, as necessary, regarding assigned clients.
- Maintains current client case record activity and progress; prepares and submits all required program reports and documentation in an accurate and timely manner.
- May participate in the staff on-call after-hours rotation process to assist clients, telephonically, with accessing higher emergent levels of treatment care.
- Adheres to all policies, laws, regulations, and codes of ethics and confidentiality as outlined by federal and state laws, and agency policies and procedures.
