SUMMARY:

Case Manager applicants must have a strong understanding of child welfare protective services and the needs of high-risk, vulnerable families. Client families are referred by the Department of Human Services due to concerns related to parenting skills, safety, and the ability to maintain children in a safe and stable environment. In this role, the Case Manager partners with families to strengthen parenting practices, enhance protective capacities, and create a nurturing home environment that promotes child safety, well-being, and permanency.

WHAT SUCCESS LOOKS LIKE IN THIS ROLE:

Success as a CUA Case Manager is demonstrated through measurable improvements in child safety, family stability, and permanency outcomes and must provide timely, consistent, and effective case management services. This includes:

  • Meeting all weekly and monthly visitation requirements for children receiving in-home and out-of-home services, ensuring compliance with DHS regulations and NCC policies.
  • Ensuring that safety assessments and service plans are completed accurately and on time along with all Key Performance Indicators set forth by DHS relevant to this role.
  • Supporting families in achieving permanency goals, including successful, timely reunification.
  • Maintaining thorough and timely documentation of case activities, including visits and service referrals.
  • Collaborating effectively with families, service providers, and the court system to remove barriers to permanency and child well-being.
  • Demonstrating consistent engagement with parents and caregivers through coaching, referrals, and support during visitation.
  • Achieving or exceeding Net Community Care’s benchmarks for child safety, permanency, and family engagement as outlined in performance evaluation criteria.
