Day Habilitation Services Support Professional
Summary:
The Day Habilitation Support Professional provides direct care to persons served ensuring safety, and excellence in care and services. The Day Habilitation Support Professional also supports their peers offering guidance and leadership. This is a part time position and considered nonexempt under FLSA. This position will be working 9am-3pm on assigned days of the week (M-F) and will be expected to cover and/or switch days with other staff members to assure that each shift is covered. To perform this job successfully, an individual must be able to perform each essential function satisfactorily. The Day Habilitation Support Professional reports to the Employment Services Coordinator.
ESSENTIAL FUNCTIONS (The requirements listed below are not intended to be an exhaustive list of all duties, or responsibilities associated with this job.):
- Provide direct supervision for all persons served while assisting with general daily living skills, social skills and/or the community as assigned.
- Provides direct and personal care for adults including but not limited to personal hygiene, assisting with bathroom needs, toileting.
- Understands and effectively implements activities geared toward achieving goals for persons served including but not limited to life skills, personal care, leisure involvement, physical/social/emotional supports.
- Assist the Day Habilitation Department in developing, implementing and monitoring of programming, including recording and reporting.
- Prepares timely and accurate documentation including but not limited to program documentation, communication logs, incident reports, seizure reports, M.A.R., and behavior documents.
- Provides positive role modeling for all persons served and peers and demonstrates appropriate professionalism.
- Provides safe transportation by adhering to transportation policies.
- Take and pass Medication Manager within the first four (4) months of employment in order to be certified in passing medications.
- Maintain status of approved driver as needed for position assigned
- Address issues that arise in a manner that adheres to OU’s policies, guidelines, practices, and procedures.
- Ensure that all organizational, local, state and federal program guideline are being met and followed.
- Maintain a clean and safe environment.
- Attend required and assigned staff meetings and trainings.
- Maintain and promote positive relationships with coworkers, persons served, families, vendors, and volunteers.
- Contribute to team effort by accomplishing assigned tasks to further mission.
- Participate on cross- functional teams.
- Advise appropriate supervisor/manager/director of any issues to ensure efficient operations, and excellent service to all persons served.
- Perform and complete unplanned duties/activities or other assignments as assigned by management.
WORKING CONDITIONS:
While performing the duties of this job, the employee is frequently exposed to indoor and outdoor conditions. While performing the duties of this job, the employee is required to frequently stand, walk, stoop, bend, sit, reach, carry, and lift. The noise level in the work environment ranges from mild to loud.
