As the Diabetic Team Lead, you will oversee individualized diabetic care plans, coordinate with physicians and caregivers, guide nursing staff, and ensure patients receive safe, consistent, and high-quality care.

At Rocky Mountain Care, we believe exceptional care starts with exceptional people. We value compassion, accountability, teamwork, and clinical excellence.

If you are a nurse who takes pride in leading others, building relationships, and improving patient outcomes, we would love to meet you!

What We're Looking For

  • Current, unrestricted Registered Nurse (RN) license in the state of practice.

  • Graduation from an accredited nursing program.

  • Two years of nursing experience preferred.

  • Home health, diabetes management, case management, or nursing leadership experience preferred.

  • Strong knowledge of diabetes care, insulin administration, and blood glucose monitoring.
