Position Overview
The Direct Service Provider / Life Skills Coach supports individuals with disabilities by promoting independence, safety, and meaningful participation in their communities. This role provides person-centered support through daily living assistance, community engagement, skill development, and documentation of services. Team members build positive relationships while helping individuals achieve their personal goals and maintain a high quality of life.
This position is located in Staten Island, NY.
Key Responsibilities
- Build positive relationships with individuals receiving services.
- Provide supervision to promote safety and well-being.
- Assist with personal care, independent living, and daily life skills.
- Support employment, education, money management, and community participation.
- Accompany and transport individuals to community activities and work sites.
- Maintain accurate daily service documentation and communicate updates to supervisors.
- Complete required trainings, staff meetings, and service documentation.
- Follow service plans, support strategies, and organizational procedures.
Requirements
- High school diploma or GED preferred.
- Previous experience working with individuals with disabilities or in a caregiving role preferred.
- Required for Day Habilitation, IRA, and Employment Support positions: Valid driver's license.
- Required for Employment Support positions: Personal vehicle.
- Required for IRA and Day Habilitation departments: CPR/First Aid certification.
- Ability to travel to off-site locations and use GPS location services within eVero when required.
- Ability to perform physical tasks, including assisting with toileting, hygiene, and lifting as needed. Reasonable accommodations will be considered.
