Direct Service Worker Case Manager

#1 In Home Care is seeking experienced, dynamic, and committed Direct Service Worker (DSW) Case Managers to join its leadership team. We’re looking for proactive professional leaders who understands home care regulations, promotes compliance and quality, and are passionate about mentoring and developing our team of caregivers. As a DSW Case Manager, you will shape a culture of teamwork built on respect and accountability — ensuring every DSW feels supported and every client receives the highest standard of care.
Key Responsibilities
- Supervise and mentor DSWs to ensure safe, effective, and compliant care.
- Conduct regular field visits and unannounced supervisory checks to evaluate quality and performance.
- Ensure accurate documentation of service notes, client records per state and federal regulations, and timesheet approvals.
- Provide coaching, performance feedback, and ongoing development support to DSWs.
- Maintain adequate staffing and provide on-call coverage when needed.
- Collaborate with clients, families, support coordinators, and other providers to ensure continuity and satisfaction of care.
- Uphold agency policies and contribute to organizational growth through innovation, accountability, and integrity.
Minimum Qualifications
- High school diploma or equivalent (associate or bachelor’s degree preferred).
- Minimum 3+ years' experience in home care, caregiving, case management, or a related people-focused role.
- Previous experience supervising, leading, or mentoring others in a home care agency, nursing home, or similar setting is strongly preferred.
- Knowledge of Louisiana Department of Health (LDH) regulations and DSW standards highly preferred.
- Excellent leadership, communication, and organizational skills.
- Proficiency with Microsoft and Google business software; LASRS experience preferred.
- Current CPR/First Aid certification.
- Valid driver’s license, reliable transportation, and proof of insurance.
- Ability to travel for site visits and work flexible hours when needed.

Physical & Work Environment
- Active daily routines include walking, driving, lifting (up to 100 lbs.), and assisting clients as needed.
- Tasks include both administrative and field work in various home and community settings.
- Requires travel and occasional work outside standard office hours.
