What You'll Do

As a Direct Support Professional (DSP), you'll provide person-centered support in our Supported Living program. You'll help individuals develop daily living skills, achieve personal goals, participate in their communities, and provide personal care as needed, all while promoting independence, choice, and a meaningful quality of life.

This full-time role is divided between supports in Rensselaer and DeMotte, allowing you to build consistent, meaningful relationships with the individuals you support.

Service Locations

Rensselaer

• Ongoing support focused on independent living skills
• Assistance with ADLs, routines, and daily reminders

DeMotte

• Weekly support, preferably on Tuesdays
• Support communication development, daily routines, and individualized goals
• Behavioral support experience is beneficial

Responsibilities

• Assist with personal care and activities of daily living (ADLs)
• Support individuals in their homes and communities
• Help build life skills such as cooking, budgeting, and household tasks
• Provide transportation to appointments and community activities
• Administer medications and monitor health needs as assigned
• Promote independence, self-advocacy, and personal growth
• Document services and communicate effectively with team members and families

What We're Looking For

• Compassionate and dependable individuals
• HHA experience strongly preferred
• DSP, CNA, caregiving, personal care, behavioral support, or related human services experience preferred
• RBT certification or behavioral support experience is a plus
• High school diploma/GED preferred but not required
• Valid driver's license and reliable transportation
• Vehicle registration and proof of insurance
• Acceptable driving record
• Ability to pass a background screening
• Strong communication skills
