Direct Support Professional - Full Time/Part Time (Lancaster/Lebanon)

SUMMARY

Provide daily support and supervision to individuals so that they may be as independent as they desire in a safe and friendly atmosphere.

ESSENTIAL JOB FUNCTIONS/MAJOR TASKS

1. Provides a caring, supportive and safe environment through observation, attending to individual needs, respecting differences and supporting growth and independence;
2. Provides comprehensive information to the team about each assigned individual through the assessment process. Offers well thought out, enhancing goal ideas;
3. Implements assigned individual program plans by carrying out community and facility based goals including supervision plans, routines, health protocols, community volunteer work and personal plans of support as written by program specialists;
4. Attends to personal hygiene and personal appearance needs including, but not limited to, changing of undergarments, outer clothing, and showering individuals as needed;
5. Attends to the health needs and changes of each individual including following diet recommendations, reporting changes to the Program Specialist, Supervisor and home caregivers. Report significant issues to the Assistant/Associate Director as well. Uses skills and information from training to respond in emergencies and to offer extra activities to individuals;
6. Provides all necessary assistance for individuals to eat including food preparation, meal modification if required, supervision and full personal support as necessary;
7. Follows all recommendations for exercise and any other protocols pertaining to the individuals including administering individual’s medication according to Medication Administration practices and policies;
8. Provides accurate and thorough documentation records in Care Tracker or other identified methods for each plan and goal. Assures that all required reports and documents are complete, accurate and submitted in a timely manner;
9. Transports individuals to community outings as required. Supports individuals to participate fully in the outing experience. Promotes individuals’ competency and encourages relationships in the community;
10. Provides ongoing supervision of individuals during program hours including breaks, lunch time, and transportation time;
11. Maintains accurate records of individual and petty cash receipts as designated by the supervisor;
12. Participates in the Incident Management Process including Risk Management through careful observation, reporting of incidents in a timely manner, and follow through of Risk Management plans to prevent future occurrences;
13. Maintains regular communication with parents, residential staff, and other professionals as designated by Supervisor. Assures that communication is done in a professional manner and encourages growth;
14. Attends meetings and trainings as required. Conducts oneself in a professional, positive, supportive manner. Assists in offering a hospitable environment in the facility when there are guests and visitors by greeting them and offering to find someone appropriate to help them;
15. Assists in the maintenance of appropriate level of supplies including paper goods, programmatic supplies, sanitary/bathroom supplies and cleaning supplies. Maintains program area and equipment (including vans) in a tidy and sanitary manner. Reports need for repairs or replacement of nonfunctional equipment immediately and to the appropriate Supervisor or the Assistant/Associate Director;
16. Fulfills chore responsibilities as assigned and helps maintain cleanliness of program and facility as needed;
17. Performs all duties and responsibilities in accordance with Excentia’s Core Values, policies and code of ethics;
18. Attends training as required. Completes a minimum of 24 hours of training relevant to human services each training year;
19. Able to think, plan, and work towards the vision of Adult Day Services;
20. Other duties as assigned by Supervisor.

The employee must frequently lift and/or move up to 25 pounds and occasionally lift and/or move more than 70 pounds with assistive devices, i.e. lift, cart, etc. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus.
