In this role, you will empower individuals by assisting with daily living activities, supporting personal goals, and promoting community involvement in a safe, positive environment.

Key Responsibilities

• Daily Living Support: Assist individuals with personal hygiene, dressing, grooming, and meal preparation.

• Plan Implementation: Follow and implement Individual Service Plans (ISP) and Behavioral Support Plans (BSP) tailored to each individual.

• Medication Administration & Documentation: Safely administer medications and accurately log daily progress, health metrics, and notes using electronic health records (EHR).

• Community Integration: Encourage personal choices, support recreational activities, and facilitate active community participation.

• Safety & Living Space: Maintain a clean, safe, supportive, and positive home environment.

Qualifications & Requirements

• Must be at least 18 years of age.

• High School Diploma or GED equivalent.

• Valid Driver’s License with a clean driving record.

• Ability to pass a mandatory state Caregiver Background Check.

• Basic computer skills (for electronic documentation and record-keeping).
