Director of Care Coordination - Program Management II

Summary

Leads the Care Coordination Program for Colorado's Behavioral Health Administration, overseeing staff and budget while coordinating transitions of civilly committed individuals from state mental health institutes and county jails to appropriate placements. Works within Colorado state government to ensure compliance with state law and rules.

This position is open to current Colorado residents only.

Located within the Colorado Department of Human Services (CDHS), the Civil and Forensic Mental Health (CFMH) is comprised of the two state inpatient acute-care psychiatric hospitals, Forensic Services, Mental Health Transitional Living Homes, Adolescent Psychiatric Residential Treatment Facility (PRTF), as well as all of the critical units that enable the work of the hospitals and other Divisions, which include: financial services, information management, Medicare/Medicaid and HIPAA compliance, hospital safety and security, and the CMHIP police.
The program this position works within Outpatient Services and will support individuals in the program who will be transferred from the department’s designated treatment locations from county jails in the State of Colorado, as well as from the Colorado Mental Health Institute in Pueblo (CMHHIP), the Colorado Mental Health Institute in Ft. Logan (CMHHIFL), to private hospital locations for inpatient Civil Commitment.

This position is remote. The work location will be your home address, with on-location work and travel for in-person meetings primarily in the Denver-metro area.

What You'll be doing:
  • Responsible for the oversight and direction of the Care Coordination Program.

  • Responsible for managing and monitoring care coordination implementation and expansion to serve Coloradans in need of placement for certifications.

  • Responsible for hiring the allocated positions to implement the program in accordance with the budget and timelines detailed.

  • Makes decisions about staff hiring, collaboration and engagement of outside entities, strategic planning for the program, budget allocation and distribution, and ongoing funding requests.

  • Program Director works closely with the administrators and staff from the Behavioral Health Administration and legislative branch to support the smooth transition of certified individuals to proper placements, and to remain in compliance with law and rule during the transfer and admissions processes.

  • Oversees the work of 18.0 FTE, including a Program Coordinator, who is responsible for primary coordination of daily operations and case managers and navigators, as well as direct/indirect oversight of the Case Managers and Clinical Navigators.

  • Oversee internal workflows, employee wellbeing, and team efficacy. This position also includes the direct supervision of 1 FTE program assistant.

  • Any additional duties that may arise while performing the job duties listed above or if needed by the division/department.

Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

PROGRAM MANAGEMENT II (H1A3XX)

Experience Only:

Nine (9) years of relevant experience in an occupation related to the work assigned to this position

Or

Education and Experience:

A combination of related education and/or relevant experience in an occupation related to the work assigned equal to nine (9) years

Special QualificationsExplain: Ability to bill under CMS. Clinical licensure in good standing including licensed professional counselor, social worker, marriage and family therapist, licensed psychologist, or addictions counselor (LPC, LCSW, LMFT, LP or LAC)


Preferred Qualifications:
  • Behavioral health leadership experience

  • Experience with Short-Term Certifications

  • Experience with data tracking, reporting, and analysis

  • Experience designing, implementing, and refining new programs

  • Detail-oriented

  • Excellent verbal and written communication skills

  • Knowledge of and experience with BHA 27-65 rules

  • Subject matter expert in serious mental illness

  • May require in-state travel to downtown Denver, CMHHIFL, and CMHHIP, and Contract Hospital Locations

CDHS Selection Process Explanation:

Employment history is calculated on a full-time basis (40/hrs per week). Part-time employment is calculated on a prorated basis to determine qualifications.
