You already know where the charts fall apart.

You've worked somewhere that found out about a documentation problem when the denial came through. Or when the surveyor asked for six records and only four were complete. You knew months earlier. You just didn't have the authority, the time, or the mandate to do anything about it.

This role is that mandate.

Tandem Behavioral Health is hiring our first Director of Clinical Compliance, a licensed clinician who owns documentation integrity and audit readiness across everything we do. Not as a side duty. Not squeezed between a caseload and supervision meetings. As the job.

What you'll actually do

Catch it the same day. Review daily documentation across every counselor, QMHP, and assessor. Missing notes, late notes, notes that won't survive a medical necessity review — you find them, name them, and send them back with the governing requirement cited and a correction deadline.

Audit charts across two levels of care. Assessments, ISPs, progress notes, crisis safety plans, discharge packets, signatures, authorizations. Outpatient ASAM 2.1 and 2.5 in Roanoke, residential crisis stabilization in Bedford. Different standards, both your responsibility.

Own the payer relationship. You're our primary clinical contact for payers. You sit in the peer-to-peer reviews. You make sure everything requested actually gets sent, complete, on time.

Lead Utilization Review. The UR Specialist reports to you. Authorization timeliness, continued stay requests, and documentation-driven denials all land in one place — yours.

Track it to closure. Deficiency rates, time-to-correction, repeat findings by program and by site. If the same gap keeps appearing, leadership hears it from you in writing before a regulator does.

Stay ready. DBHDS licensing inspections, MCO record reviews, payer audits. Continuous readiness, not a scramble.

The hybrid arrangement, stated plainly

Chart review, audit tracking, reporting, and analysis happen remotely. Signature verification, source document reconciliation, and anything requiring the physical record or the care environment happen on site.

You'll keep a regular scheduled presence at both Roanoke and Bedford, set with the VP of Clinical Operations and adjusted around audit activity, census, and survey cycles. When a licensing inspection or payer audit lands, you're on site.

The sites are about an hour apart. Travel between them is part of the job, mileage reimbursed or gas card provided. Remote work requires a private, HIPAA-compliant workspace and agency-approved devices.
