Director of Population Health & Care Management (4130)
Essential Responsibilities
Population Health Leadership
- Develop and execute the organization’s Population Health strategy in alignment with WFM’s clinical strategies
- Establish priorities for improving health outcomes across defined patient populations.
- Lead initiatives to improve preventive care, chronic disease management, and care gap closure.
- Oversee patient registries, risk stratification methodologies, and outreach strategies.
- Monitor and improve organizational performance on UDS, HEDIS, value-based care, and other quality measures in collaboration with the Director of Clinical Quality, Safety, and Risk.
- Evaluate the effectiveness of population health interventions and recommend improvements.
Care Management Leadership
- Provide executive oversight of the Care Management Supervisor.
- Ensure effective delivery of care management services for high-risk and medically complex patients.
- Oversee programs including:
- Care Management (nursing and social work)
- Children and Pregnant Women (CPW) programs
- Community Health Workers (CHWs)
- Transitional Care Management
- Chronic Disease Management
- Promote standardized workflows and evidence-based care management practices.
- Ensure care management resources are aligned with organizational priorities.
Value-Based Care
- Collaborate with executive leadership on value-based care strategy.
- Support implementation and optimization of value-based payment programs.
- Monitor contractual quality and utilization performance.
- Identify opportunities to improve performance under value-based payment arrangements.
- Collaborate with payer partners on quality improvement initiatives.
Population Health Analytics
- Lead development and use of population health dashboards in collaboration with Informatics
- Translate clinical and claims data into actionable operational strategies.
- Monitor trends in utilization, quality, access, and patient outcomes.
- Support physician and operational leaders through meaningful performance reporting.
- Utilize data to prioritize organizational improvement initiatives.
Leadership and Collaboration
- Develop annual departmental goals, budgets, and staffing plans.
- Recruit, mentor, and develop high-performing leaders and staff.
- Foster collaboration across clinical departments and operational teams.
- Serve as a resource to physicians and clinical leaders regarding population health strategies.
- Partner closely with the Director of Clinical Quality, Safety & Risk to ensure alignment between quality improvement and population health initiatives.
Regulatory Compliance
- Ensure compliance with applicable federal, state, HRSA, and payer requirements.
- Maintain knowledge of evolving value-based care and population health models and best practices.
- Support organizational readiness for regulatory reviews and audits.
