We are seeking an experienced and compassionate Director of Social Services to join our leadership team. This full-time leadership position plays a vital role in enhancing the quality of life for residents by overseeing social services, resident advocacy, care coordination, and discharge planning within our long-term care community.
$40.00-45.00 hourly rate plus $5,000 Sign on Bonus!

The ideal candidate is an experienced long-term care professional who is passionate about resident-centered care, skilled in navigating complex healthcare needs, and committed to supporting residents and families throughout their journey.

What You'll Do

As Director of Social Services, you will:

• Lead and oversee all social services operations within the facility.
• Champion resident rights, dignity, independence, and quality of life.
• Coordinate discharge planning and care transitions to promote positive outcomes.
• Partner with interdisciplinary care teams to ensure comprehensive, person-centered care.
• Conduct psychosocial assessments and develop individualized service plans.
• Provide support, counseling, education, and guidance to residents and family members.
• Connect residents and families with community resources and support services.
• Facilitate care plan meetings, family conferences, and resident advocacy initiatives.
• Ensure compliance with federal, state, and professional standards.
• Maintain accurate and timely documentation and records.
• Contribute to quality improvement efforts and organizational excellence.
