Pay Range $53.71 - $60.00 Per Hour (Depending on Years of Experience)

We are currently seeking a Full-Time RN Discharge Planner to join our team at BCBIC (Baltimore Central Booking & Intake Center) located in Baltimore, Maryland.

The RN Discharge Planner is responsible for coordinating patient discharge planning from a correctional setting. The Discharge Planner will work under the direction of the Director of Nursing in collaboration with a multidisciplinary team to develop and implement a safe and appropriate discharge plan to promote continuity of care after release.
