POSITION SUMMARY:

Develops a training and learning environment to support adults with developmental disabilities acquire job skills and advance their careers. Designs instructional methods based on clear, measurable objectives, tracks client progress, analyzes learning barriers, and creates alternative instructional plans to address these barriers. May involve traveling to clients’ homes, work sites, or community settings, or providing remote services. Coordinates individualized service delivery and manages resource development to meet all client-centered needs. Provides a range of services to adults with developmental disabilities, including but not limited to:

  • Volunteer & Paid Work instruction
  • Employment Placement & Support Services
  • Mobility Training
  • Sensory Integration Activities
  • Social Activities in the community
  • Academic Activities
  • Behavior Plans
  • Leadership Activities
  • Community Involvement (self-advocacy)
  • Aging Support Services
  • Independent Living Skills

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Direct Support Responsibilities:

  • Works with clients individually or in group settings to provide a wide range of services to overcome barriers to employment and to keep meaningful jobs.
  • Develops and implements personalized coaching plans for clients, both in preparation for work and at their place of employment.
  • Plans, implements and coordinates clients' overall program plan relative to employment.
  • Addresses issues and crises related to client services promptly and effectively.
  • Provides ongoing advocacy and follow-along support for implementation of each client’s individual plan.
  • Stakeholder & Community Interaction:
  • Act as the onsite expert and primary contact for employer partners.
  • Act as a liaison to families, circle of support, employers, case managers and community resources.
  • Assist in creating and maintaining community sites and partnerships.
  • Attends and participates in relevant meetings, conferences and training as assigned.

Reporting & Quality Assurance:

  • Collaborates with service manager(s) to prepare annual calendar for program participant reviews, assists in facilitating case reviews and goal planning for each program or service.
  • Keeps up-to-date employer contacts and emergency information.
  • Maintains accurate records of client emergency information and client records.
  • Maintains documentation and records accurately and timely, including Individual Service Plan information and goals tracking, assessments, daily/weekly/monthly progress reports.

Others:

  • Ensures adherence of all safety policies and procedures.
  • Attend all required meetings and trainings
  • Duties as assigned.
