Coordinate active ageing programmes, social activities, and community outreach for seniors to prevent isolation and promote wellness.
Job Summary
The role involves proactively engaging clients, organising social activities and encouraging an active lifestyle to prevent social isolation. The position also entails referring clients to relevant community care and social assistance when needed. Under the Age Well SG initiatives, the role requires collaboration with primary care physicians (e.g., family doctors) to empower residents in taking charge of their journey toward healthier and more active lifestyles.
Responsibilities
- Conduct outreach and engage seniors in the community.
- Plan and run active ageing programmes and activities.
- Identify seniors’ needs and connect them to appropriate services.
- Coordinate referrals and follow-up with community partners.
- Recruit and engage volunteers and senior volunteers.
- Build partnerships with community and healthcare organisations.
- Maintain records, monitor KPIs and prepare reports.
- Support centre operations, events and service improvements.
- Undertake additional duties as assigned by the Manager or Executive Director to support organisational goals
Preferred competencies and qualifications
- Diploma/Degree in a relevant discipline.
- Experience in eldercare, community work or programme management preferred.
- Good interpersonal, communication and organisational skills.
- Able to engageseniors and work independently and in a team.
