FACT LPN

Sign-On Bonus: $2000

• Oversees medication administration.
• Coordinates with psychiatric and medical providers.
• Provide rehabilitation and support functions to assist in treatment, medication education, support, consultation to families, and crisis intervention.
• Individual and group interactions with individuals who have a serious and persistent mental health disorder.
• Community-based with services provided in the home or other community settings.
• Document all activities in the Electronic Medical Record (EMR).
• Provides case management services.
